Department Introduction With the sanctioned faculty strength of eight and senior residents of sixteen, today the Advanced Urology Center is the largest recognized center for comprehensive training in M.Ch Urology and one of the busiest urology services in India. The post-doctoral residency program includes full-fledged training in most areas of Urology and Urodynamics, Andrology, Female Urology, and clinical relevant exposure in Pediatric Urology and renal transplantation. Over 115 surgeons have been trained in Urology (M.Ch) so far; all of them are doing well and many hold prestigious positions at national and international levels. Despite the ever increasing workload (over 50% in last 5 years; fig 1, 2). The department has strived hard and has been able to provide up-to-date clinical services, and contributed significantly to Medical research. Since inception, the department took keen interest in conducting in-depth basic and clinical research in areas relevant to Urology keeping in mind the common problem in our country. The following account depicts the development of this super-specialty training and service facility from beginning to present state. 1960s Urology services began as a Division in the Department of Surgery in 1966 with Late Dr. P.N. Kataria and Dr. Inder Prakash as the Assistant Professor and Senior Lecturer, respectively. The first M.Ch Urology fellow Dr. K.L. Kurisakose joined in January 1967 and completed training in January 1969. 1970s 1971 Due to untimely demise of Dr. P.N. Kataria in June 1971 there was a void in the department which was filled by Dr. B.C. Bapna who took charge of this Division in September 1971. He was joined by Dr. M.S. Rao in 1972 and Dr. S.K. Sharma in 1975. 1978 The division of Urology became a full-fledged department of Urology with 25 elective indoor beds. 1979 Dr. B.C. Bapna was the Founder Member, Honorary 1980 Secretary and President of the Urological Society of India. He was also a fellow of National Academy of Medical Sciences. 1970 The first urodynamic facility in the country was established in the Institute. 1980s The Department had been running the most popular and structured M.Ch Urology training program in the country with the capacity of 8 post-doctoral trainees and 4 faculty members. The lower tract Endourology services were fully established. The department was providing service in urinary stone disease, benign prostatic hypertrophy post-traumatic urethral strictures, genitourinary cancers, genitourinary tuberculosis (GUTB), genitourinary fistulae and congenital genitourinary malformations. In addition, the department started services in Neurourology & Urodynamics and Male Infertility & Sexual Dysfunction. The workload increased tremendously due mainly to its quality treatment and care. In face of rising number of cancer patients and availability of supportive facilities like CT Scanning, Angiography and Radio-interventions. Pathology and Radiation Oncology, the department took a leap in Uro-Oncology and started performing major curative operations for urological cancers (e.g. radical cystectomy with continent urinary diversions, radical Nephrectomy and retroperitoneal lymph node dissection etc.) Besides patient-care, the department, since its inception, took major initiative for basic and clinical research relevant to the need of our country(e.g. urinary stone disease, voiding dysfunctions in spinal-cord injury patients, GU tuberculosis and GU fistulae, etc.) in collaboration with departments of Biochemistry, Pharmacology, Experimental Medicine, Biophysics of PGIMER, Punjab University, Chandigarh, ICMR, CSIR and other funding agencies. This resulted in large number of publications in reputed indexed scientific journals, some of which ar3e widely cited and referred-to in various Journals and textbooks. Significant research contribution in Urolithiasis included study of physical, bacteriological and electron microscopic characteristics of staghorn calculi, molar lactate infusion therapy for uric acid urolithiasis, effect of diclofenac sodium upon urinary excretion of calcium, uric acid, creatinine and PGE2 in patients of nephrolithiasis etc., in Uro-Oncology included study of circadian biology of prostatic acid phosphatase and in Neuro-Urology included pharmacokinetic modulation of vesico-urethral function in patients with neurogenic bladder dysfunction, role of endorphin in pathophysiology bladder dysfunction, role of endorphin in pathophysiology of destrusor areflerxia in traumatic paraplegics/quadriplegics during spinal shock phase and the effect of suprostone (Prostaglandin) upon detrusor function in patients with neurogenic vesical dysfunction. 1981 Dr. S.K.Sharma was selected for a Common Wealth Fellow to UK (August, 1981 to September, 1982). 1985 Dr. M.S. Rao joined at K.M.C Manipal as Professor of Urology. Dr. S.Vaidyanathan and Dr. A.K.Goswami joined as Assistant Professor at this Institute. 1986 First radical cystectomy with ileal conducit, and augmentation cystoplasty were performed in the department for invasive bladder cancer and GUTB, respectively. 1987 Dr. B.C. Bapna superannuated and Dr. S.K.Sharma took over as Head, Department of Urology. 1988 Dr. A.K. Mandal joined as Assistant Professor. One post of Assistant Professor of Urology for the Department of Emergency was sanctioned. 1989 The Department organized a National Workshop on Ureterorenoscopy and Percutaneous Nephrolithotomy (PCNL) which set the stage for Upper Tract Endo-Urological Surgery. 1990s 1990 First continent-cutaneous urinary diversion was performed in a patient with classic exstrophy. 1991 The department organized an International Workshop on “Penal prosthesis and Uro-Oncology”. 1992 · Dr. S.K. Sharma was awarded Fellowship of the National Academy of Medial Sciences of India. · Dr. S. Vaidyanathan left the department for U.K. 1993 · Dr. S.K. Singh joined as Assistant Professor. · Dr. A.K. Goswami was awarded WHO Fellowship on Endourology at Long Island Jewish Hospital, New York. · The department organized 3rd Annual Conference of North Zone Chapter of Urological Society of India. · Dr. S.K. Sharma took as Medical Superintendent – cum- Professor of Hospital Administration. · Dr. A.K. Mandal was awarded Member travel-Fellowship of USI on Infertility. 1994 The department acquired Lithoclast with pneumatic energy for management of stone disease. 1995 Dr. S.K. Sharma became Honorary Secretary of the Urological Society of India. 1996 · Dr. S.K. Sharma was awarded Dr. B.C. Roy National Award of MCI for establishing a discipline of Medicine. · The department started supporting services for management of chronic renal failure with introduction of Arterio-Venous Fistula formation and insertion of Continuous Ambulatory Peritoneal Dialysis (CAPD) catheter placement. · Dr. Uttam K Mete joined as Assistant Professor. · Ultrasound was procured for hands-on training of the senior residents. 1998 · 31st Annual Conference of Urological Society of India was organized. Dr. P.N. Kataria Memorial Oration and Chandigarh Best Video Prize during each annual conference of North Zone Chapter of Urological Society of India and Urological Society of India respectively were introduced. The “Trust for Education, Research & Development of Urology” was registered. · Dr. A.K. Mandal was awarded WHO fellowship in the field of Male Reproductive Medicine at Cornell Medical center, New York, USA. · Lithotripsy services were introduced in August 1998 and with this the department became a center for comprehensive management of urinary stone disease with all facilities (URS, PCNL, ESWL & open surgery for complicated cases) under one roof. · Dr. S.K. Sharma received various awards of the Urological Society of India i.e. from Orations to President’s Gold Medal and the Urology Gold Medal. By this time the department had earned the reputation of being the most popular referral center for management of complicated urological problems north of Delhi and was ready to impart training in various subspecialty areas of Urology. The existing staff strength of five full-time faculty members and 10 M.Ch trainees was inadequate to stream line the service for the overwhelming workload. A clear need was palpable for expansion and up gradation of the department. 2000s 2000: Dr. S.K. Sharma was elevated as the Director of the Institute, nominated to the Senate of Punjab University Chandigarh and became the Dean of the Faculty of Medical Sciences of Punjab University, Chandigarh. 2002 · Dr. S.K. Sharma was awarded Member Travel-Fellowship of USI and he availed the same for Laparoscipic Renal Surgery including Donor Nephrectomy at SGPIMS, Lucknow. · Dr. S.K. Sharma took the charge of the office of President of USI. · Dr. A.K. Mandal took the charge of the office of Honorary Secretary of North Zone Chapter of USI. 2003 · The Ministry of Health & Family Welfare, Govt. of India granted approval for upgradation and expansion of the department of Urology to Advanced Urology Center during the 10th Five Year Plan. · Urological Laparoscopy services were introduced in the department with Laparoscopic Nephrectomy. 2004 · Dr. A.K. Mandal took over as the Head of the Department. He was awarded Millenium International Urology Congress travel Fellowship of NZ Chapter of USI. · Dr. Santosh Kumar joined as Assistant Professor and he was awarded Fellowship of the Royal College of Surgeons of Edinburgh (FRCS). · The Advanced Urology Centre was inaugurated on September 22, 2004 by Hon’ble union minister of Health and Family Welfare. The indoor-bed strength was augmented to 60, including 6-bedded postoperative recovery room. The operation theater in the Lithotripsy Center was made operational. · Orthotopic Neobladder reconstruction (following Radical Cystectomy) was introduced. 2005 · Dr. A.K. Mandal was promoted as the Professor of Urology. · A live-operative workshop was organized to introduce 100W Holmium: YAG laser for management of BPH, stone disease, stricture and bladder tumor. · An advanced digital Video-Urodynamic Machine with facility of Nerve-conduction studies was installed. · An Operating Surgical Microscope was procured and microsurgical urological procedures like vaso-epididymal anastomosis, vasectomy reversal and varicocoelectomy etc. were introduced. · Dr. Mayank Mohan Agarwal joined as Assistant Professor and he was awarded Membership of the Royal College Surgeons of Edinburgh(MRCS) · Dr. V.Chandra Mohan was awarded Gold Medal in DNB Genitourinary Surgery. 2006 · Dr. A.K. Mandal delivered Dr. Pinnamaneni Venkateswara Rao Oration during the USICON-2006. · Dr. S.K. Singh was selected as Professor of Urology. He was awarded Milenium International Urology Congress travel Fellowship of NZ Chapter of USI. · Dr. Naveen Chandra Acharya joined as Senior Research Associate. · Advanced laparoscopic procedures (Radical Nephrectomy, Pyeloplasty, Nephron-Sparing Surgery, Pelvic lymph node dissection, etc.), procedures in Female Urology (minimally invasive sling operations for stress urinary incontinence, urethral reconstruction with anterior bladder flap and by transvaginal approach, corrective procedures for genital prolapsed) and in Uro-Oncology (Radical Retropubic Prostectomy for prostate cancer) were introduced. · Dr. S.K.Singh was elected as a member of Governing Body of Indian Council of Medical Research (ICMR), New Delhi. · International Instructional Course and live workshop on Good Urodynamics Practice was organized. · Dr. Naveen Chandra Acharya joined as Assistant Professor he was awarded Membership of the Royal College of Surgeons of Edinburgh (MRCS). · Dr. Mayank Mohan Agarwal was awarded Gold Medal DNB Genitourinary Surgery. 2007 · The Govt. of India sanctioned approval for additional staff (faculty, senior residents, paramedical and supporting staff) for Advanced Urology Center. With this the faculty strength increased to 8 (including one post for Assistant Professor of Anesthesia) and senior resident to 17 (including one port for Anesthesia) · Dr. Nitin Gupta joined as Senior Research Associate. · High-resolution 3D USG equipment with facility of Doppler and Urological Interventions including intraoperative laparoscopic transducer was installed. A high-power digital Radio- fluoroscopic Unit for Uro-radiological investigations and interventions was installed. · The department organized an International Live-Operative Workshop on Advanced Urological Laparoscopy.
